Information we collect and how we use your information:

We collect the information about you when you register your interest with us or contact us by completing the enquiry form on
our website or email us with an enquiry. We collect information about you to answer your query and to offer appropriate help and/ or intervention where indicated. We will not share your information with anyone else without your permission.

We use Google analytics to monitor the use of the website. Google analytics use cookies to help analyse how people use our
website and this information may include your IP address and the pages you visit. You may refuse the use of cookies by using your browser settings.

• Contact we have with you such as appointments
• Relevant information from people who care for you such as health and social professionals, relatives or carers.
• Notes and reports about your health treatment and care
• Results of tests such as blood test results, ECG,S, XRAY and scans
• Financial information, such as invoices, and payments

Your personal information

In order to provide psychiatric services we collect information to assess your health and decide on the most appropriate care for you. The information is also used to improve the quality of care you receive.

Please note that we cannot guarantee confidentiality in email correspondence. If you send us personal information online or by email, we will presume that this indicates permission to email personal information back in response to your enquiry by email. If you have concerns about sharing information online, you can send the information to us by post. Please see our postal address below. Information you share with us online or by post may be saved with your medical record.

Sharing your information

We have a duty under the Health and Social Care Act 2012 to share information about you with other health and social care professionals directly involved in your care so that you receive the best quality care. E.g. we may share information about your diagnosis, treatment, medications and test results. Your GP and other health and social care professionals will have a responsibility to keep your information confidential, and they will be able to tell you how they ensure confidentiality of your information.

We may share this information with other health care professionals who are directly involved in your care to ensure you receive safe and best possible treatment/ care. We will only do this when either they have a genuine need for it or we have your permission.

We will not disclose information about you to third parties without your permission, unless your or somebody else’s health and safety are at risk, or the law requires us to pass on information.

Storing of your information

We do not keep your records for longer than necessary, and are guided by medical professional standards. We usually keep the records for 8 years after which the records will be destroyed securely.

Your legal rights:

You have the right to request a copy of the information that we hold about you. You can get a copy by emailing or writing to us.

 Under GDPR you have a right to –

• Request access to information we hold about you.
• Request correction of any information we hold if the information is inaccurate.
• Request erasure of your personal information if you feel that there is no purpose to
  hold information about you.
• Request restriction of processing information we hold about you.

Sometimes there may be legal reasons as why we cannot do this, in which case we will explain to you why we are not able to do
so.

Access to your personal information

If you want to know what personal data we have about you, you can ask us for details and request a copy if you wish. This is known as “subject access request”. All requests should be made in writing and sent to the email address or postal address given below. You need to have proof of identity, authority to act on someone else’s behalf, if this is appropriate, and give us some indication what information you are seeking.

There is a nominal charge for this, if your request is excessive for instance the request is repetitive, further fees may be charged to meet the administrative costs.

We aim to respond to your request within 28 working days of receipt of your request. If your request is complex more time may be required up to a maximum of 3 months. You will be kept informed of progress.
